We are looking for a Full-time Registered Nurse (RN) for a Hospice Clinical Care Coordinator position in the home setting.  The Registered Nurse must have at least 2 years of current or very recent hospice experience. Leadership / Management experience in any healthcare setting is preferred but is not a requirement. This position is often called the Director of Clinical Services in many home hospice agencies. The Director of Clinical Services will lead the clinical team, coaching individual team members and leaders in the organization.  The Hospice Clinical Care Coordinator will lead QAPI (Quality Assurance and Performance Improvement) efforts of the organization.  The Clinical Care Director will be responsible for clinical outcomes of two separate sites as well as patient/family education and satisfaction efforts.  Role combines administrative office functions as well as being a role model for the clinical team through patient/family education on disease progressions, what to expect for patient/family and collaborative partners (physicians, hospitals, nursing facilities, assisted living and independent living communities). The Hospice Director of Clinical Services should be open and show the ability to be willing to model clinical expectations and hold team members to exceptional care giving habits and discipline. Requirements of the Hospice Clinical Care Coordinator / Director of Clinical Services Registered Nurse position: The Registered Nurse must have at least 2 years of current or very recent hospice experience.
