Overview

We’re hiring a Part-Time Registered Nurse for an Intake Coordinator role with a long-established and respected Home Health and Hospice agency. This role offers a consistent weekday schedule, great pay, and the opportunity to work in a meaningful, patient-focused role.

 

Schedule:

  • 3 weekdays per week, 9:00 AM to 5:00 PM

  • 30-minute lunch

  • You choose which 3 weekdays (Mon–Fri), but they must remain the same each week

About the Role:
The Intake Coordinator manages and facilitates patient transitions from hospital to home. You’ll act as a key liaison between the agency and physicians, hospital discharge planners, social workers, and community health referral sources. You’ll maintain accurate, timely records and report to the Director of Intake and Service Development to support scheduling, quality improvement, and follow-up efforts.

 

Compensation & Perks:

  • Salary: $47,500 – $60,000/year, based on experience

Requirements:

  • RN license in the state

  • At least 1 year of experience in a certified home health agency

  • U.S. Citizen or Green Card holder (no exceptions)

  • COVID-19 vaccination required (or willing to obtain if hired)
