Overview

We are seeking a Full-Time Registered Nurse (RN) for a Hospice Case Manager position in the home health setting. This role offers exceptional flexibility — you schedule directly with your patients, allowing you to start and end your day around your personal needs and appointments. While hospice experience is preferred, we welcome applicants with at least 1 year of recent experience with adult or geriatric medical/surgical patients. You will manage a caseload of hospice patients and provide compassionate end-of-life care in the comfort of their homes. This position is with a well-established and stable home health and hospice agency that has served the community for many years.

Requirements:

  • Registered Nurse (RN) with active license in the state

  • See multiple patients per day in homes (some drives may be 20–40 mins)
  • U.S. Citizen or Green Card holder

  • At least 1 year of experience with adult or geriatric medical/surgical patients
