We are looking for a full-time Home Health & Hospice Transitional Care Liaison / Marketer for a position with a home health and hospice agency. You will be joining a team of two other liaisons.

We are seeking a licensed practical nurse (LPN) who is outgoing, well spoken, has strong problem-solving skills, and a background in customer service.

The Home Health & Hospice Liaison will be responsible for building and maintaining relationships in the community. This includes marketing to physicians, skilled nursing facilities, and other healthcare partners. The company has been in business for many years, providing stability and long-term job security.

Requirements:

• Active LPN license in the state with at least 2 years of treating experience 

• Willing and able to work full-time hours

• Outgoing, well spoken, problem-solving, and customer-service focused
