We're hiring a Home Health Physical Therapist – Baylor Shift


Enjoy full-time pay and benefits while working just three days a week! This role follows a Baylor schedule: 10-hour shifts on Saturday, Sunday + either Thursday or Friday. Get the flexibility you want with the stability you need.

 

 

What You’ll Do:

  • Conduct in-home physical therapy evaluations and create individualized treatment plans

  • Perform OASIS assessments and complete timely documentation

  • Supervise and collaborate with PTAs and other clinical team members

  • Deliver evidence-based treatments to help patients regain mobility, strength, and independence

  • Educate patients, families, and caregivers on treatment plans and safety practices

  • Participate in case conferences and coordinated care planning

  • Maintain accurate clinical records and follow Medicare/compliance guidelines

  • Support transitions of care and refer to other services as needed

  • Communicate with physicians and internal agency team consistently

Requirements:

  • Licensed Physical Therapist (Bachelor’s, Master’s, or Doctorate in PT)

  • Licensed to practice in the state

  • Minimum of 2 years of clinical experience (home health preferred)

  • U.S. citizen or Green Card holder
