We are seeking a Supervisor of Clinical Services to oversee our interdisciplinary clinical team in Home Health Care. This leadership role involves planning, organizing, and guiding staff to deliver effective, patient-centered care. The Clinical Manager supervises, supports, and educates staff directly in the field while ensuring compliance and quality.

Pay & Benefits:

What You’ll Do:

• Oversee all patient care services and personnel during operational hours.

• Lead an interdisciplinary team managing a patient census of ~200.

• Ensure adequate coverage for weekdays, weekends, and holidays.

• Interview, hire, and onboard salaried, per diem, and contract staff.

• Support and guide field staff through patient calls, triage, and follow-up.

• Oversee patient assessments, care coordination, and plan of care updates.

• Collaborate with QAPI and Education to support staff growth and OASIS documentation quality.

• Hold clinicians accountable for agency standards, timely documentation, and productivity goals.

• Provide role modeling, direct observation, and performance evaluations.

• Participate in policy design, quality improvement, and compliance activities.

Requirements:

• Licensed Registered Nurse (RN), Physical Therapist (PT), or Occupational Therapist (OT) in NY

• Bachelor’s degree in Nursing, Physical Therapy, or Occupational Therapy

• At least 2 years of recent Medicare Home Health experience 

• Some management experience in any healthcare setting

• U.S. Citizen or Green Card holder (no exceptions)

• COVID-19 vaccine required
