We are seeking a compassionate Registered Nurse (RN) to join our team as a Hospice Case Manager. This is a full-time, weekday position providing care to patients in their homes. While hospice experience is preferred, we will consider RNs with at least 3 years of experience caring for adult or geriatric medical/surgical patients.

Requirements:

• Active RN license in the state

• Preferred: Hospice experience
