Overview

We are seeking a Full-Time Registered Nurse (RN) for a Hospice Case Manager position in the home health setting. This is a Monday through Friday role (8:30 AM – 4:30 PM) with significant flexibility, as you coordinate directly with your patients to schedule visits. Requirements:

  • Registered Nurse (RN) with an active license in the state

  • At least 1 year of recent experience working with adult/geriatric med-surg patient

  • U.S. Citizen or Green Card holder
