Overview

We are hiring a compassionate Hospice Registered Nurse (RN) Case Manager to provide care to patients in their homes throughout St. Louis, St. Charles, and Jefferson counties. This is a full-time Monday–Friday (8am-5pm) role with flexibility to schedule directly with your patients so you can plan your day around your needs.

Requirements:

  • RN license in the state of Missouri

  • US Citizen or Green Card holder

  • Preferred: Hospice experience

  • Will consider RNs with 2+ years recent experience with adult/geriatric med-surg patients

  • Must participate in on-call rotation (not just triage; in most cases, you will need to go out and do a visit) :
    • One weekend per month (day and night)
    • 1–2 weeknights per month on call from 5:00 PM to 8:00 AM.
      • Call volume may range from 1 call per day to 8 calls per day.
      • Must be able to respond in person while on call (those who take sleep aids that impair responsiveness cannot be considered).
