We’re seeking a Full-Time Registered Nurse (RN) for a Hospice Admissions / Float role. This position is Monday–Friday, 8:30 a.m.–5:00 p.m., with flexibility based on patient needs.

Responsibilities:

• Complete accurate and timely hospice admissions.

• Assess patients for hospice eligibility and appropriate level of care (routine, GIP, continuous care).

• Review charts and gather supporting documentation (labs, radiology, hospital records, medications, POA, Living Will, physician orders).

• Participate in on-call rotation as needed.

• Cover visits across territories as assigned (routine, emergency, death visits, level of care changes, etc.).

Requirements:

• Active RN license in the state

• U.S. Citizen or Green Card holder

• Minimum 2 years of medical, surgical, oncology, or hospice nursing experience
