We are seeking a Full-Time Registered Nurse (RN) for a high-paying, permanent position in a residential home environment. The Registered Nurse will supervise and assist with the coordination of all aspects of health care for individuals served within our residential homes in collaboration with the treatment team. This role ensures that individuals achieve their highest level of health and wellness while overseeing the clinical operations of the home.
Primary Responsibilities:
Communicate and collaborate with Program Directors to review medical records, current medications, health status, and functional abilities consistent with ISP and Annual Physical requirements.
Hire, train, schedule, and supervise LPNs.
Ensure staff receive required medical and clinical trainings.
Identify learning needs and develop or secure appropriate trainings for staff at all levels, including RNs, LPNs, direct care staff, and program leadership.
Act as an advocate and liaison for individuals to ensure they receive care consistent with clinical best practices.
Case Management Responsibilities:
Conduct comprehensive nursing assessments of individuals’ health and psychosocial needs.
Develop and maintain Case Management, Medical, and Nursing Care Plans according to ISP timelines.
Provide episodic case management for hospital admissions and discharges.
Assist with reviewing referrals and conducting intake assessments for new residents.
Collaborate with individuals, families, and caregivers to develop medical records, specialized protocols, and data collection systems.

Active Registered Nurse (RN) license in Massachusetts
Experience with tracheostomy (Trach) and ventilator (Vent) care is required
Must be a U.S. Citizen or Green Card holder
