We are seeking a Full-Time Registered Nurse (RN) for a high-paying, permanent position in a residential home environment. The RN will supervise and coordinate all aspects of health care for individuals served within our residential homes in collaboration with the treatment team. This role ensures each individual achieves their highest level of health and wellness while overseeing the home’s daily clinical operations.
Primary Responsibilities:
Communicate and collaborate with Program Directors to review medical records, medications, and health status.
Hire, train, schedule, and supervise LPNs.
Ensure staff receive required medical and clinical trainings.
Identify learning needs and provide appropriate training for RNs, LPNs, direct care staff, and program leaders.
Act as an advocate and liaison to ensure individuals receive quality, person-centered care.
Case Management Responsibilities:
Conduct comprehensive nursing assessments of health and psychosocial needs.
Develop and maintain Case Management, Medical, and Nursing Care Plans according to ISP timelines.
Provide case management for hospital admissions and discharges.
Assist with new resident referrals, intake assessments, and specialized protocols.
Collaborate with families and caregivers to support care coordination.

Requirements:
Active Registered Nurse (RN) license in Massachusetts
Minimum three years of nursing experience providing direct care to elderly or disabled adults
Must be willing to drive to various locations for home visits and medical appointments
Must be a U.S. Citizen or Green Card holder
