We are seeking a Full-Time Physical Therapist (PT) for a very flexible position in home health care.

Summary of Responsibilities:

• Travel within Thurston County to provide intermittent in-home care.

• Follow physician orders and plans of care under the direction of the Director of Clinical Services.

• Collaborate with other home care personnel in patient care planning.

• Direct and supervise the Physical Therapy Assistant and Home Health Aide, as assigned.

• Conduct initial patient evaluations and histories.

• Consult with physicians regarding changes in treatment.

• Instruct patients and families in home programs and activities of daily living.

• Participate in Quality Assessment and Performance Improvement activities.

• Complete and submit OASIS assessments, reassessments, transfers, resumptions of care, discharges, and significant change in condition documentation.

Requirements:

• Bachelor’s, Master’s, or Doctorate degree in Physical Therapy

• Some home health experience required

• Must be a U.S. Citizen or have a Green Card
