We are seeking a Full-Time Physical Therapist (PT) for a very flexible position in home health care.
Summary of Responsibilities:
Travel within Thurston County to provide intermittent in-home care.
Follow physician orders and plans of care under the direction of the Director of Clinical Services.
Collaborate with other home care personnel in patient care planning.
Direct and supervise the Physical Therapy Assistant and Home Health Aide, as assigned.
Conduct initial patient evaluations and histories.
Consult with physicians regarding changes in treatment.
Instruct patients and families in home programs and activities of daily living.
Participate in Quality Assessment and Performance Improvement activities.
Complete and submit OASIS assessments, reassessments, transfers, resumptions of care, discharges, and significant change in condition documentation.

Requirements:
Bachelor’s, Master’s, or Doctorate degree in Physical Therapy
Some home health experience required
Must be a U.S. Citizen or have a Green Card
