We are seeking a Full-Time Physical Therapist (PT) for a very flexible home health position serving adult and geriatric patients. This role offers independence, strong support, and good pay.
Evaluate patients referred by physicians using diagnosis-specific assessments
Provide in-home PT visits for adult and geriatric patients
Complete timely documentation (evaluations, progress notes, time & mileage)
Schedule and manage your daily patient visits
Collaborate with physicians and interdisciplinary team members
Communicate patient progress and updates as needed

Bachelor’s, Master’s, or Doctorate in Physical Therapy
Active PT license in the state
Minimum 2 years of PT treating experience
Must be a U.S. Citizen or Green Card holder (no exceptions)
Comfortable with driving between patient homes (Most patients are close together; occasional 20–40 min drive. Mileage reimbursed.)
