Overview

We are seeking a Full-Time Physical Therapist (PT) for a very flexible home health position. This role offers meaningful one-on-one patient care, manageable territories, and excellent compensation—without on-call requirements.

Key Responsibilities:

  • Provide physician-ordered physical therapy services in patients’ homes

  • Evaluate patient function and develop individualized plans of care

  • Complete initial evaluations, reassessments, and plan-of-care updates every 30 days

  • Supervise Physical Therapy Assistants and Home Health Aides as appropriate

  • Educate patients and caregivers on exercises and home care routines

  • Collaborate with physicians and the interdisciplinary care team

  • Complete timely and accurate clinical documentation, including OASIS

  • Participate in quality initiatives, in-services, and care conferences

Requirements:

  • Bachelor’s, Master’s, or Doctorate degree in Physical Therapy

  • Active Physical Therapy license in the state

  • Valid driver’s license and reliable transportation

  • Must be a U.S. citizen or Green Card holder (no exceptions)
