We are seeking a Full-Time Physical Therapist (PT) for a very flexible home health position. This role offers meaningful one-on-one patient care, manageable territories, and excellent compensation—without on-call requirements.
Provide physician-ordered physical therapy services in patients’ homes
Evaluate patient function and develop individualized plans of care
Complete initial evaluations, reassessments, and plan-of-care updates every 30 days
Supervise Physical Therapy Assistants and Home Health Aides as appropriate
Educate patients and caregivers on exercises and home care routines
Collaborate with physicians and the interdisciplinary care team
Complete timely and accurate clinical documentation, including OASIS
Participate in quality initiatives, in-services, and care conferences

Bachelor’s, Master’s, or Doctorate degree in Physical Therapy
Active Physical Therapy license in the state
Valid driver’s license and reliable transportation
Must be a U.S. citizen or Green Card holder (no exceptions)
1. This is a full-time position. Are you willing and able to work full-time hours?

3. This is not shift work. The role involves seeing multiple patients per day in their homes or senior living communities. You will drive between visits—most patients are close together, but occasionally drives may be 20–40 minutes. Traveling is part of the role, and mileage is paid. Are you comfortable with this?
