We are hiring a Full-Time Hospice RN for a home health setting. In this role, you will care for patients in senior living communities and private homes.

What You’ll Do:
Provide hospice nursing care in patient homes and senior living communities
Manage caseload and coordinate plans of care
Perform skilled nursing treatments and symptom management
Support patients and families with compassionate end-of-life care
Educate caregivers and coordinate with the interdisciplinary team
Travel between nearby patient locations throughout the day

Requirements:
At least 3 years of nursing experience
Active RN license
Hospice or home health experience preferred
Strong compassionate communication style
Comfortable building relationships with patients and families
Positive, supportive bedside manner
Willing and able to travel between patient homes and communities
